    • Consent: By voluntarily providing your mobile phone number (e.g., on intake forms or during registration), you consent to receive these healthcare-related text messages. This consent is not required as a condition of receiving care or services. Messages may be automated or manual.

    • Risks: Standard SMS messages are not encrypted and may be intercepted. We limit sensitive details in texts to protect your privacy, but we recommend using our patient portal (once available) for detailed health information.

    • Opt-Out: You may stop receiving texts at any time by replying "STOP" (or "END," "CANCEL," "UNSUBSCRIBE," or "QUIT") to any message. You will receive one final confirmation message. You may also email staff@jonmd.us to opt out.

    • Help: Reply "HELP" to any message for assistance, or contact us at staff@jonmd.us or 832-534-3802.

    • Charges: We do not charge for messages, but standard message and data rates from your carrier may apply. Participating carriers include major U.S. providers (e.g., AT&T, T-Mobile, Verizon); compatibility varies.

    • Changes/Updates: Notify us immediately of phone number changes via staff@jonmd.us. We may modify or discontinue texting services at any time.

    • Maintain privacy and security of PHI.

    • Notify you of any breach affecting unsecured PHI.

    • Abide by this notice (we may change it; updated versions posted here and available in-office).

  • We retain information as needed for treatment, payment, operations, or legal requirements. We use reasonable safeguards (e.g., encryption, access controls) to protect data.

  • Our services and website are not directed to children under 13. We do not knowingly collect their information.

  • Material changes will be posted here with a new effective date.
